    Abstract
  •   Objective  The prevention and control of human immunodeficiency virus (HIV) in China still faces significant challenges. Currently, men who have sex with men (MSM) have become one of the key populations in efforts to reduce HIV transmission. To control HIV transmission among MSM, pre-exposure prophylaxis (PrEP) serves as a crucial preventive measure. However, the uptake of PrEP among MSM in China remains low. This study aimed to explore the barriers to PrEP use among MSM through qualitative interviews.  Methods  Following the principle of information saturation, 10 MSM were recruited from an HIV-focused non-governmental organization in Beijing between April and August 2023 for in-depth interviews. A qualitative research approach using 7-step thematic analysis was employed to summarize and extract key themes from the interview data.  Results  A total of 10 MSM participated in the study, with ages ranging from 21 to 53 years. Six participants had received higher education, and six reported a monthly income of 5 000-10 000 RMB. Through data analysis, 10 subthemes were identified: awareness level, publicity, prevention of other infectious diseases, passion, behavioral disinhibition, drug side effects, dosing regimen, drug accessibility, cost, and benefits. These subthemes were further consolidated into three overarching themes: cognitive factors, sex and drug-related factors, and economic factors.  Conclusion  The comprehensive implementation of PrEP in China still encounters considerable challenges. Chinese MSM exhibit insufficient knowledge of PrEP, concerns regarding its limited scope of disease prevention, memory lapses in medication compliance, fear of side effects, and high costs. It is recommended to promote PrEP through multiple channels, enrich educational content, and strengthen peer-led initiatives. Additionally, the role of MSM-focused NGOs should be leveraged to enhance awareness of infectious disease prevention and mental health support within this population. Harnessing the influence of these organizations may accelerate the dissemination and adoption of PrEP strategies.
