Objective During recent years,the use of a number of free flap method in the breast cancer surgery,the flap thickness also have different discussion.We tried to make surgery done thoroughly,often worried thick flap may cause residual tumor,affecting local recurrence and distant metastasis after surgery.While ignoring the complications of breast cancer.This article further investigate the effect of different thickness of the flaps on the postoperative efficacy and complications of radical breast cancer.
Methods The breast cancer patients were randomly divided into thin flap surgery group,medium thickness skin flap surgery group for comparison research.In addition to the two groups of patients during surgery using the different methods of the flap thickness,the rest is the same.
Results In the 87 cases of free thin flap group,5 cases of locoregional recurrence,the recurrence rate of 5.7%,5-year survival of 62 cases,5-year survival rate was 71.3%,17 flap necrosis,severe necrosis of the six cases;localized area effusion 39 cases,22 cases occurred area effusion duration of the delay.In the 99 cases of thick free flap group,six cases of locoregional recurrence,the recurrence rate of 6.1%,5-year survival of 75 cases,5-year survival rate was 75.8%,4 flap necrosis,severe necrosis 1 case;localized area effusion 21 cases,six cases occurred area effusion duration of the delay.The distant metastasis and local recurrence rates in the two groups had no statistical difference,and the partial necrosis rate of skin flap and the duration of the incidence of local effusion,regional effusion were statistically significant differences(
P<0.05).
Conclusion The different thickness of skin flaps had no obvious effectradical breast cancer in postoperative distant metastasis and local recurrence rate,medium thickness skin flap can effectively lower the local skin flap necrosis rate and incidence of local effusion,shorten the duration of the regional fluid.
