Effect of Ursodeoxycholic acid combined with S-Adenosylmethionine in the treatment of intrahepatic cholestasis of pregnancy

摘要: 目的 探讨熊去氧胆酸(UDCA)联合S-腺苷蛋氨酸(SAM)在妊娠期肝内胆汁淤积症(ICP)治疗中的临床价值。 方法 将德阳市人民医院2010年1月—2015年10月间收治的134例ICP患者作为观察对象,按随机数字表法分为观察组(OBG组)66例和对照组(COG组)68例,OBG组给予UDCA和SAM治疗,COG组给予常规治疗,比较2组患者肝脏生化指标和分娩情况。 结果 采用UDCA和SAM联合治疗的66例OBG组患者中,自然分娩34例(50.75%,χ2=8.562,P=0.019),人工助产4例(11.76%,χ2=9.025,P=0.014),剖宫产33例(49.25%,χ2=8.747,P=0.026),胎儿存活67例(100%,χ2=8.637,P=0.009),OBG组患者治疗后TBil(t=16.562,P=0.035)、ALP(t=18.935,P=0.024)、γ-GT(t=19.025,P=0.015)、TBA(t=19.365,P=0.012)和ALT(t=16.253,P=0.035)等肝脏生化功能指标均恢复正常,与COG组比较差异存在统计学意义(P<0.05)。 结论 UDCA联合SAM治疗ICP能够显著改善患者的肝功能指标情况,提高自然分娩和良性妊娠的几率,改善患者预后,值得临床推广借鉴。

Abstract: Objective To investigate the effect of ursodeoxycholic acid(UDCA) combined withs-adenosylmethionine(SAM) in the treatment of intrahepatic cholestasis of pregnancy(ICP). Methods A total of 134 patients in the Department of Obstetrics and Gynecology between January 2010 and October 2015 were taken into this research and were divided into OBG(66 patients)and COG(68 patients)group according to random number table.The patients in OBG group received UDCA and SAM,while the COG group received the conventional treatment.The biochemical markers of liver and delivery conditions in two groups were compared. Results In OBG Group,there were 34(50.75%)cases of spontaneous labor(χ2=8.562,P=0.019),4(11.76%)cases of artificial aiding-birth(χ2=9.025,P=0.014),33(49.25%) cases of caesarean section(χ2=8.747,P=0.026),67(100%)fetal survived(χ2=8.637,P=0.009).The levels of TBil(t=16.562,P=0.035),ALP(t=18.935,P=0.024),γ-GT(t=19.025,P=0.015),TBA(t=19.365,P=0.012) and ALT (t=16.253,P=0.035) after the treatment in OBG Group recovered,which were significantly better than CTG Group(P<0.05). Conclusion Ursodeoxycholic acid and s-adenosylmethioninein the treatment of intrahepatic cholestasis of pregnancy can obviously improve thebiochemical markers of liver,promote the healthy pregnancy and natural childbirth,and improve the prognosis of patients,which is worthy of clinical use.
