多病共存患者治疗负担相关评估工具的研究进展

Recent advances in tools for assessing the treatment burden of multimorbidity

  • 摘要: 随着老龄化进程加快,慢性非传染性疾病患病率逐年增加,多病共存现象越来越普遍。多病共存患者的健康管理是全球卫生保健服务的重要挑战。相较于健康、患单一慢性病的人群,多病共存患者依赖更多的医疗资源,在影响个体生活质量的同时, 给家庭及社会带来沉重的照护压力和经济负担,加重治疗负担。治疗负担是指患者因为医疗保健需承担的工作及其带来的影响。患者会经历不同程度的治疗负担,了解治疗负担,识别并确定最有可能承受高治疗负担的人群,设计满足他们需求的医疗保健服务非常重要。美国、法国、英国等西方发达国家对治疗负担已有相关评估工具并得到推广应用,准确、有效评估治疗负担的工具有利于患者知晓自身健康状况及相应的挑战,为制定个性化干预措施、评价干预效果提供依据,积极的医学干预对减轻患者治疗负担有显著的积极作用。本文就多病共存患者治疗负担相关的评估工具进行总结,介绍了治疗负担的概念,治疗负担问卷、多病共存感知量表、日常用药负担问卷、医疗保健任务困难量表、患者关于治疗和自我管理的体验量表、药物负担相关的生活质量量表、慢性病共存治疗负担问卷、老年慢病共存患者治疗负担量表等主要评估工具的内容、信效度、优缺点,以期总结现有的经验和模式, 因地制宜创新适宜本土文化的多病共存患者治疗负担的评估工具, 为医务人员选择相关评估工具提供参考。
