The expression and significances of PD-1 and PD-L1 in peripheral blood T cells of patients with acute exacerbation of chronic obstructive pulmonary disease

摘要: 目的 探讨慢阻肺急性加重期(AECOPD)患者外周血T细胞中程序性细胞死亡蛋白-1(PD-1)和其配体(PD-L1)的表达水平及与临床指标和预后的关系。 方法 以2017年5月—2018年5月温州市中心医院接收诊治的79例AECOPD患者和30例健康体检者进行研究,利用流式细胞技术检测外周血CD4+、CD8+ T细胞中PD-1、PD-L1的表达水平,分析其表达与临床指标的关系,采用Cox回归模型分析影响此病预后的因素。 结果 与健康体检者相比,AECOPD患者外周血CD4+ T细胞明显升高,CD8+ T细胞明显下降(P<0.05)。AECOPD患者外周血CD4+、CD8+ T细胞中PD-1和PD-L1的表达水平高于健康者外周血CD4+、CD8+ T细胞中PD-1和PD-L1的表达水平(均P<0.05)。CD4+、CD8+ T细胞中PD-1、PD-L1表达水平与患者吸烟、咳痰量、支气管感染、气短轻重、PaO2、PaCO2、治疗依从性、使用抗氧化剂等均相关(均P<0.05)。吸烟、咳痰量、支气管感染程度、气短轻重、PaO2、PaCO2、是否使用抗氧化剂以及PD-1和PD-L1表达是影响AECOPD患者预后的独立因素(均P<0.05)。 结论 AECOPD患者外周血CD4+、CD8+ T细胞中PD-1和PD-L1表达水平升高,且与临床指标及预后密切相关,推测PD-1和PD-L1可能参与AECOPD患者的发生发展。Abstract: Objective To investigate the expression of PD-1 and PD-L1 in peripheral blood T cells of patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD) and its relationships with clinical indicators and prognosis. Methods A total of 79 patients with AECOPD and 30 healthy examiners in our hospital from May 2017 to May 2018 were enrolled as the subjects. The expression levels of PD-1 and PD-L1 in peripheral blood CD4+ T cells and CD8+ T cells were detected by flow cytometry, and the relationships with clinical indicators were analyzed. The prognostic factors were analyzed by Cox regression model. Results Compared with healthy subjects, CD4+ T in peripheral blood of patients with AECOPD increased significantly, CD8+ T decreased significantly(all P<0.05). The expression levels of PD-1 and PD-L1 in AECOPD patients were higher than that of healthy people(P<0.05). The expression levels of PD-1 and PD-L1 were correlated with smoking, expectoration volume, bronchial infection, shortness of breath, PaO2, PaCO2, and antioxidant use(all P<0.05). Smoking, expectoration volume, degree of bronchial infection, shortness of breath, PaO2, PaCO2, antioxidant use or not, and the expressions of PD-1 and PD-L1 were independent prognostic factors in AECOPD patients(all P<0.05). Conclusion The levels of PD-1 and PD-L1 in peripheral blood CD4+ T cells and CD8+ T cells are increased in AECOPD patients, and they are closely related to clinical indicators and prognosis. It is speculated that PD-1 and PD-L1 may be involved in the occurrence and development of AECOPD patients.
